Cory Monteith, 31 Years Old
The "Glee" star was sent to rehab towards the end of the show's final seasons due to a heroin addiction. He died of an accidental overdose in 2013. He was dating costar Lea Michele at the time, and following his death, Ryan Murphy organized a tribute episode that tied both Monteith and his character's deaths together.
